Custom Tool Bar

I am having difficulty adding a new ICON to an existing toolbar, can anyone assist?

Probably, but we would need a more detailed description of your problem.  Exactly what are you trying to do?  Is this adding a tool to the toolbar or just changing an icon for an existing tool?  What have you tried?
