I've noticed that when the manipulator is in the back corner of object you can't get hold of the grip to move it, everything else gets in the way, so you first have to spin it around to select the white move grip, or is there a selection cycle method.

In the situation you describe, right-click the Manipulator and select the Move option from the menu that appears.
The 'Move' option is the first option in the menu to enable fast selection...

OK thanks, right clicking was taking me straight to extrude, but that's because I was still hovering over the object grip and that was taking precedence, just moving the mouse slight before right clicking so it is on the manipulator brings up the menu.
